Primary Care & Hope Clinic accepts TennCare, Medicare, and most commercial insurance plans. In addition, we have a sliding fee discount program for qualifying patients and offer Telehealth visits.

How to schedule an appointment

Interested in being a Primary Care & Hope Clinic patient? Please call (615) 893-9390 to schedule an appointment at any location. Prior to your appointment, please print and complete the New Patient Information Form and bring it with you to your appointment.

How to complete all forms and paperwork

Once you have downloaded and completed all forms and paperwork, be sure to bring them with you to your scheduled appointment. For best practice, it is a good idea to keep your new patient forms in a place you will remember before you attend your scheduled appointment. How to attend your appointment

Once your appointment is set and you have completed all paperwork, be sure to add the date of your appointment to your calendar. We ask that all patients arrive 15 minutes before your scheduled appointment time with all paperwork, insurance, and prescription information.

Sliding Fee Discount Program

We offer a sliding-fee scale discount to all eligible patients, based on annual household size and income.

 

All items are required for intake appointment or appointment will be rescheduled.

 

  • Photo ID: Driver’s License or other state-issued ID
  • Proof of residence
  • Proof of income (can be provided by letter, bank statement or copy of check):
    • 3 current check stubs
    • Copy of most recent tax return
    • Unemployment stub
    • Pension and/or Retirement
    • Social Security; Disability; or SSI
    • Child Support
    • Letter from employer stating hours worked & amount earned per week
    • Recent employer separation letter
How to schedule an appointment

To qualify for the sliding fee discount, patients must complete the social services intake process to establish their sliding scale fee. Patients must re-qualify for their sliding fee discount each year.
