The Primary Care and Hope Clinic serves as a medical home for patients in Middle Tennessee, regardless of their ability to pay. Through the consistent delivery of medical and behavioral health services, the clinic ensures that our patients receive the care they need to get healthy and stay healthy.
Who we serve
The Primary Care & Hope Clinic is committed to being a provider of choice for all patients in Middle Tennessee, regardless of their ability to pay or insured status.
That means that, in addition to offering a sliding fee to low-income uninsured patients, many of our patients have Medicare, Medicare Advantage, TennCare, and most commercial insurance plans.
Our patients range in age from 0 to 100 years, and our staff can treat a variety of needs from preventive care, immunizations, well-child checkups, urgent care illnesses, and chronic diseases.

Who we are
We are a modern, well-equipped health center staffed by physicians, nurse practitioners, licensed clinical social workers, a dietitian, psychiatrist, and clinical pharmacists. Our staff follows evidence-based guidelines to ensure the most appropriate, patient-centered treatment.
Operating in Rutherford County since 1992, the Primary Care & Hope Clinic has grown from a staff of five to over one hundred employees in four sites across three cities. Three of those initial five employees are still providing leadership and direction to the health center today, ensuring the quality of care grows with us.
