All copays, sliding fees or cash payment options are due at check-in. No one will be denied access to services due to inability to pay.

Primary Care & Hope Clinic utilizes a third-party lab for outside laboratory services and has an agreement to provide discounts based on income and household size for our uninsured and underinsured patients. Lab fees will be billed directly from the lab.

Sliding Fee Program

Primary Care & Hope Clinic welcomes all patients regardless of insurance status. We offer financial assistance through a discounted fee program. By Federal Law, qualification is based on household size and income. You will be asked to complete an application and provide proof of income for all adult members in your household. The completion of those documents will determine if you are eligible for the program, the category your household size and income place you in, and the fee(s) required at time of service.

Admittance into the sliding fee program is valid for a period of one year. Your fees could change if your income or household size increase or decrease during that time. Please inform us if this occurs. Primary Care & Hope Clinic trusts that the information you provide is true and accurate. We reserve the right to disqualify you from the sliding fee program if you have misrepresented information.

Primary Care & Hope Clinic understands that sometimes financial circumstances make it difficult to pay for your visit at the time of service. If you are experiencing hardship, you may contact our office for assistance. No one will be denied access to services due to inability to pay.

Billing and Insurance Information

Primary Care & Hope Clinic provides excellent care for everyone regardless of their insurance status. We welcome TennCare (Medicaid), CoverKids, Medicare and most commercial insurance plans in addition to a self-pay program for uninsured patients. We are not responsible for knowing what your medical and behavioral health insurance benefits are and recommend you call your insurance company to find out if Primary Care & Hope Clinic is included in your plan benefits.

If your appointment is for a physical (“check-up”, “routine care”, “wellness exam”), your insurance may or may not cover your visit. If you consent to being seen, you are responsible for any charges your insurance does not cover.  Please read our Wellness Exam Policy  for more information.
