Visiting a new clinic doesn’t have to be challenging. The Primary Care & Hope Clinic accepts TennCare, Medicare, and most commercial insurance plans. In addition, we have a sliding fee discount program for qualifying patients and offer Telehealth visits.

Sliding Fee Discount Program

We offer a sliding-fee scale discount to all eligible patients, based on annual household size and income.

All items are required for intake appointment or appointment may be rescheduled.

Photo ID: Driver’s License or other state-issued ID
Proof of income (may include any of the following that apply):
- 3 current check stubs
- Copy of most recent tax return
- Unemployment stub
- Pension and/or Retirement
- Social Security, Disability, or SSI
- Child Support
- Letter from employer on company letterhead stating hours worked & amount earned per week
- Recent employer separation letter
