Care Coordination
Sometimes, a patient might need services we don’t provide.
PC&HC utilizes staff who work alongside your providers as part of your care team to ensure we can link you with the help you need within our community.
- Assistance with enrolling for health insurance.
Certified Application Counselors (CACs) are responsible for assisting patients with eligibility for TennCare, CoverKids, and Marketplace insurance applications.To speak with one of our CAC’s, call 615-893-9390 and select option ‘5’.
- Supporting the care teams
Patient Navigators serve by assisting the provider and nurse when patients need durable medical equipment or home health, and work with the care team in closing gaps in care.
- Referrals and scheduling with outside specialists
Our Care Coordinators connect patients with outside specialists, scheduling the visits according to patient insurance and preferences, and follow-up to ensure our providers are up to date on changes to the treatment plan.They can also help connect patients and families with community resources for help with food, transportation, and housing.
Patients should speak with their provider, who will connect them with the right staff member for their needs.
