Through the consistent delivery of medical and behavioral health services, the clinic ensures that our patients receive the care they need to get healthy and stay healthy.

Primary Care & Hope Clinic serves as a medical home for uninsured and underserved patients in Middle Tennessee, regardless of their ability to pay. Through the consistent delivery of preventative, acute, chronic, and behavioral health services, the clinic ensures patients receive the care they need to get healthy and stay healthy. Primary Care & Hope Clinic is staffed by physicians, and certified nurse practitioners, behavioral health consultants, and a registered dietitian.

The Primary Care & Hope Clinic provides healthcare for the entire family and all age groups. Primary medical services include annual physicals, women’s health services, immunizations, treatment of minor injuries, and management of chronic conditions. 

The Primary Care & Hope Clinic uses integrated and collaborative health care approaches to provide whole-person care.  We believe that the best care is given when the entire team of health care providers works together to treat the overall health of the patient.  No person is the same and your health care should reflect that. Our behavioral health team includes therapists, psychiatrists, psychiatric nurse practitioners, and case managers who work together for ongoing therapy, resources, and medication management. 

For many patients, medications are an important part of their treatment, and critical to maintaining their health. The cost of those medications shouldn’t be a barrier to care. Luckily, the Primary Care & Hope Clinic operates a full pharmacy, utilizing the discounts of the federal 340(b) program to provide those prescriptions at dramatically reduced prices.

Living a healthy lifestyle should be the goal of every patient, and this starts with diet and nutrition. Consulting with a dietician provides many benefits as it can help you maintain your weight goals, but it will also help you create plans to help with many health-related issues such as diabetes, blood pressure and more.

The Primary Care & Hope Clinic is committed to being a provider of choice for all patients in Middle Tennessee, regardless of their ability to pay or insured status.

That means that, in addition to offering a sliding fee to low-income uninsured patients, many of our patients have Medicare, Medicare Advantage, TennCare, and most commercial insurance plans.

Our patients range in age from 0 to 100 years, and our staff can treat a variety of needs from preventive care, immunizations, well-child checkups, urgent care illnesses, and chronic diseases.
